What is organised for students on commencement of placement?

Initial meeting to establish goals, competencies and how these can be achieved.

What are the arrangements for mentors/assessors?

You will be allocated a Supervisor / Assessor prior to commencing your placement and be offered learning opportunities with all the staff whilst you are with us. All our Practitioners have been trained with the latest updates and you will have access to each Practitioner so to best shape your experience.  

What shift patterns are students allocated for learning?

Our service operates 7 days per week, including Bank Holidays. We work between the hours of 8am & 6pm including weekends. We do not work outside of these hours.

What patient care situations are available in this placement?

Our service aims to provide an alternative to hospital admission for the older adults of the Bury borough where safe to do so. We form part of the acute care pathway, adopting a multi-disciplinary approach to our service provision which will typically last no longer than 6 weeks.

During working hours, we are responsible for all older adult admission requests and follow the gatekeeping process for this. We conduct mental health assessments, risk assessments, care planning, engage in inter-professional working, and explore continued professional development to better equip our practice.    

What nursing model is used for planning care?

We are currently involved in a pilot scheme to look at new ways to approach person-centred care planning. Utilising the DIALOG+ approach is evidenced to enable healthcare professionals to have supportive and meaningful conversations with service users about their physical and mental health, in addition to other aspects of their life that are most important to them, such as family, relationships, leisure activities, and accommodation.
