About this placement

What is organised for students on commencement of placement?

What are the arrangements for mentors/assessors?

Assessors and supervisors attend workshops to develop their skills to support students in practice

What shift patterns are students allocated for learning?

Early:

07.30-14.35. Break is 20 minutes (total of 6.75 hours)

Late:

Late 13.25-20.30. Break is 20 minutes (total of 6.75 hours)

Night:

20.15-07.45. Break is 50 minutes (total of 10.66 hours)

Shift Notes:

Long day 07.30-20.30. Break is 1 hour (total of 12 hours)

Notes: in the hub staff work 3 long days a week. The spoke practitioners work Monday to Friday 9-5.

What patient care situations are available in this placement?

Bed Management is made up of a senior practitioner and a bed manager together they have an overview of demands on beds and available capacity across the GMMH footprint. They gatekeep admissions, assessing and prioritising need. They liaise on a daily basis with A&E departments, community teams, Home treatment teams, ward liaison, AMHP services and the spoke practitioners to ensure patients are appropriately placed in a timely manner taking in to consideration individual need.

Spoke practitioners work within each of the inpatient units, they have an overview of their bed area and ensure that admissions and discharges are facilitated smoothly and without delays. They update bed management twice a day in conference calls on any discharges happening within their area. They also work closely with the community teams to ensure the correct package of care is in place to ensure safe discharge

What core clinical skills can be learnt?

Assessment skills, risk assessment and management

→ Observing mental health risk assessments with the spokes

→ Gain experience of multi disciplinary team working and multi agency working
→ Observe and take part in supervised evidence based mental health nursing practice

→ Gain experience in working with service users to identify possible barriers to discharge in     order to prevent prolonged admissions.

→ taking part in mental health risk assessments (including documentation of this)
→ Observing brief psychosocial intervention

→ Working with your practice supervisors to coordinate assessments and MDT discussions around discharge planning.

→ Referral and signposting to services, allowing development of knowledge of services available

→ Multi disciplinary team working and multi agency working

→ Development of documentation and communication skills (demonstrating this on Paris)
→Taking active part in evidence based mental health nursing practice

→ Exploration of the impact of psychosocial stressors on mental health and exploring this with the patient under supervision

→ Working within the mental health act including client on Section 2 MHA, CTO recalls/revocations, and emergency uses of the act such as Section 5(2) and 5(4). Including sitting with clients and explaining their rights under MHA.

→ Taking part in completing risk assessments and formulating discharge/follow up plans

What additional clinical skills can be learnt?

→ Completing mental health risk assessments with your practice supervisors, including use of appropriate questions and documenting accurately

→ Taking the lead as shift coordinator under supervision of practice supervisors.
→ Brief psychosocial intervention
→ Mental health promotion and education

→ Referral and signposting to services, allowing development of knowledge of services available

→ Multi disciplinary team working and multi agency working, identifying and referring to appropriate services

→ Care planning, development of documentation and communication skills

→ Exploration of the impact of psychosocial stressors on mental health

→ Working within the mental health act including client on Section 2 MHA, CTO recalls/revocations, and emergency uses of the act such as Section 5(2) and 5(4).
