Additional Information

PCMHWS provides emotional, health education, social and community support to those who do not meet the threshold of secondary care mental health services. It provides easy access for GP’s and patients who are too complex for IAPT or GP only care, but who are subthreshold for current secondary care providers. Thus, bridging the gap with the provision of a “middle tier”.  We focus on patients with high levels of unmet need with a diagnosis of depression and anxiety disorders, as well as those with long term physical health conditions who would benefit for mental health support. We do so via NHS, Social Care and Third Sector partnership-working to provide a more holistic model of assessment and support.
