About this placement
What is organised for students on commencement of placement?
Students will be allocated a Practice Supervisor and Practice Assessor prior to placement. Dependent on where their supervisor in based that will be where the student will be based. They will introduce them to the team and complete their initial assessment. A welcome pack will be made available to students who provide an email address prior to placement or a pack will be given to them on the first day.
What are the arrangements for mentors/assessors?
Students will be allocated 1 Assessor and 1 Supervisor prior to placement. This can be found on PARE and in the welcome pack.

Hours can be adjusted according to spoke placements or on discussion with Assessor
What patient care situations are available in this placement?
WE aim to achieve the best possible outcome by working closely with the patient, family, carers, therapist and other members of the multidisciplinary team (MDT) to improving the patients’ health and wellbeing.
The community therapy team offer a short term programme of rehabilitation in the patient’s own home.
Prevent unnecessary acute hospital admission or long term care
Maximise independent living and reduce the need for home support
Support people with health conditions, frailty and multiple co- morbidity
What nursing model is used for planning care?
Holistic assessment completed and care plans are generated to address the person’s needs.
What core clinical skills can be learnt?
Working as part of MDT, opportunity to work closely with physiotherapist, occupational therapist and other members of the multidisciplinary team in the person’s own home.

What additional clinical skills can be learnt?
Communication, Infection control, Observations, wound management, nutrition assessments and hydration management. Pain management., Bowel Management, Basic wound care and pressure prevention and care.

Internet, Books, student board, Policies and procedures files, library. Student forums, library-hospital
What research and practice development activities are being undertaken?
Actively encourage students to question clinical practice. Take part in team activity and discussion
Spoke Placements
Please see student board and welcome pack for the full list. Spokes include: Community therapists (Physio and Occupational therapists), District nurses, Dieticians, Bladder and bowel team. Podiatry, Admission Avoidance team, Speech and language therapy, Tissue viability.
Additional Information
The Health and Care nursing team work alongside all members of the MDT to complete holistic assessment of the person’s needs so that they maintain their independence at home and improve their quality of life.
