About this placement
What is organised for students on commencement of placement?

We work 7 days a week:-
08.30am – 17.00pm
What patient care situations are available in this placement?
We are nurse assessors and arrange care to be in place before discharge.
We assess for a safe and timely discharge
We also deal with palliative patients and their families in regards to their preferred destination and help them arrange this so we deal with very sensetive situation
What nursing model is used for planning care?
Roper, Logan and Tierney and the Nursing Process
What core clinical skills can be learnt?
We are nurse assessors so we are involved in clinical assessment of patients in regards to appropriate pathways for patients such as nursing assessments and intermediate care, the discharge process, palliative care in the community (arranging a safe discharge) – options available and limitations,assessment of equipment needed – so assessing the patients clinical need for that equipment and looking at the safety aspects – especially in regards to bed rails in the community.
What additional clinical skills can be learnt?
We work across all boundaries from acute to community so it’sdeveloping the knowledge and the ability to link all the services up so thepatient receives a good quality service – continuity. During the studentsspokes they will be with all different teams and disciplines – so they will beable to see and learn different skills from them. We are the team everyonecomes to if they don’t know what to do in regards to discharge so they will seea wide range of complex needs that we have to problem solve, we are also theremost of the time as the patients advocate. We deal with a lot of patients whohave dementia and lack capacity – so they will learn about the process whenthat happens and dolls and why that is used.
What resources are available to help students learning?
The students have access to the knowledge service all ourpolicies a selection of nurses from different backgrounds and knowledge,extensive spokes within our service and other services.
