About this placement

What is organised for students on commencement of placement?

  • We will email a welcome pack prior to the student commencing, that will provide insight of the service and the varying roles of the MDT
  • We will allocate the student an assessor/supervisor prior to commencing placement
  • We will discuss spoke placement opportunities and rotation to other intermediate care sites

What are the arrangements for mentors/assessors?

We have nursing staff that are both assessors and supervisors and will ensure that PARE documentation is complete timely at every stage of your placement and ensure competences that are discussed at initial meeting are met and signed off.

What shift patterns are students allocated for learning?

Shift Notes:

The working full time rota is three long days worked over the seven-day week: 08:00-20:30

If the shift pattern is not suitable for the student, then reasonable adjustments can be discussed.

What patient care situations are available in this placement?

  • We assess patients medically, physically, and socially and refer to appropriate MDT members so that ongoing long-term issues highlighted in the assessment process can be managed in the community setting.
  • Care and treatment interventions can be provided by the Rapid Response service for up to seventy two hours and if ongoing care is required after this time the patient would be referred and excepted by the wider MDT to take over care provisions.
