We aim to provide a high quality health and social care service to meet the holistic care needs of people in Oldham with a functional or organic illness. We provide person centred care to people over the age of 65 and also provide care for people with an early onset dementia under the age of 65.

We work with people in a variety of different settings – in people’s homes, residential and nursing homes and outpatient’s clinics.

Carers play a vital role in the support of our patients and the team work in accordance with the Care Act ensuring that their needs are met and assessed and that support is provided on a regular basis.
