Night:  19:30-08:00

Days: 07:30 – 20:00

Ward for patients with complex medical needs awaiting for discharge to care homes or care packages.

What nursing model is used for planning care?

Roper Logan and Tierney

  • Personal care
  • Nutrtion and hydration
  • administration of medications
  • Observations, early warning scores and escalation if required
  • IV administration
  • Complex discharges
  • MDT meetings
  • Working with members of allied health professionals
  • Venepuncture
  • Communication skills
  • Catheter care may include insertion.
  • Observation of enhanced care
