What is organised for students on commencement of placement?

Shadowing an assessor/supervisor to learn what the job entails.

What shift patterns are students allocated for learning?

08:00-16:00 Monday-Friday

Notes:
10:00-18:00 can be arranged with an allocated worker

What patient care situations are available in this placement?

Assess and plan what care/support is required for discharge. This includes fast track end of life discharges, rapid end of life discharges, assessing patient’s who require packages of care at home, assessing patient’s who require residential or nursing care. Attending best interest meetings to discuss discharge and complex cases. Attending board round to learn from the MDT.

What nursing model is used for planning care?

The discharge to assess model is used in the home first acute discharge team, and the home first model. We explore all options to try and get the patient home safely. If this is not possible, we use the discharge to assess model to get temporary placement in a care home to allow the patient’s needs to be assessed further.
